An 81-year old man was admitted to the University Campus Bio-medico of Rome for a second opinion after diagnosis of melanoma of the bladder in December 2014. His medical history included arterial hypertension, hypothyroidism treated with L-thyroxine, peripheral arterial disease treated with by pass, and corneal transplantation at the age of 74. His past medical history was negative for malignant melanoma. He experienced gross hematuria in the absence of any other symptoms and the urine cytology identified atypical cells. Patient underwent transurethral resection (TUR) of the bladder with diagnosis of melanoma. We performed another endoscopic resection of a 14 mm lesion of the anterior bladder wall that was identified at the total-body Computed tomography (CT) of January 1st 2015 [Figure 1].
