Background: EBUS-STAT can be used to score and classify EBUS-TBNA operators from novice to expert. Furthermore, simulator use leads to rapid acquisition of clinical EBUS-TBNA skills comparable with that of conventional training methods on patients.We evaluated the efficacy of EBUS-TBNA training with a simulator using the complete and partial EBUS-STAT items.Methods: 15 physicians attending the post graduate training program in Interventional Pulmonology at the University of Florence were assessed at baseline by 3 evaluators by administrating the EBUS-Stat during a high fidelity virtual reality simulation. After that, a theoretical session on systematic assessment of mediastinum was carried on, followed by a 3 hours practice with the simulator under experienced supervision and, finally, by re-administration of the EBUS stat.Differences between the EBUS-stat scores at baseline and after training were detected. To discriminate the pounded role of items in the technical skills subgroup, global, and partial scores were compared.Results: Significant improvement was seen by the global score (p=0.004).We found no significant differences in the score assessing the practical skills (p=0.134) whilst a significant increase in the EBUS and CT scan recognition scores was observed (p=0.002).To test the effect of a basic knowledge of flexible bronchoscopy,we re-evaluated the differences after excluding the first two items and found that mean scores of items 3-7 were different (p<0.001).Conclusions: virtual reality simulation improves trainees confidence and ability in EBUS-TBNA and the EBUS-STAT questionnaire items 3-10 better assesses improvement of skill in intermediate trainees.
