The management of chronic ruptures of the tendo Achillis often requires augmentation techniques. Open procedures on the Achilles tendon can lead to difficulty with wound healing because of tenuous blood supply and increased chance of wound breakdown and infection. This article describes a minimally invasive method of the reconstruction for the Achilles tendon using a free autologous semitendinosus tendon graft. This technique helps to preserve skin integrity over the site most prone to wound breakdown, and can be especially used to reconstruct the Achilles tendon in the presence of a gap greater than 6cm.
