Introduction: Nurses usually think that their assessment of patients’ health is attentive and in accordance with that of the patients. However, various studies have pointed out some discrepancies in the perception of health problems between nurses and patients. As this is a little-known phenomenon in Italy, a study was carried out in the University hospital Campus Bio-Medico in Rome, aimed to assess the patients’ problems and the degree of concordance between the problems felt by patients and those recognized by nurses, and to identify factors that affect concordance. Methods: The descriptive study utilized a purpose-built questionnaire, and given to the patient and the nurse caring for him in order to assess the patient’ health problems at the admission in a ward of Campus Bio-Medico hospital. Results: The research, carried out on 55 patients and 12 nurses, pointed out that the degree of concordance between patients and nurses in the perception of patients’ problems is on average 39,6% and it is greater for physical problems (51%) rather than for psychological (37%) and social problems (31%). Discussion: Patient’s perception of his own health problems is a very important factor for the understanding, the cooperation and the involvement in his plan of care. Also nurse’ assessment of a patient’s problem affects the depth, the range and the quality of the plan of care. The results of the study provide interesting suggestions for clinical practice, education and nursing research.

Introduzione: Gli infermieri generalmente ritengono che la loro valutazione sullo stato di salute dei pazienti sia accurata e congruente con quella dei pazienti stessi. Tuttavia numerosi studi hanno messo in evidenza delle discrepanze nella percezione dei problemi di salute tra infermieri e pazienti. Poiché il fenomeno in Italia è poco conosciuto si è deciso di effettuare uno studio presso il Policlinico Campus Bio Medico di Roma con l’obiettivo di valutare i problemi presentati dai pazienti e il grado di concordanza tra i problemi percepiti dai pazienti e quelli identificati dagli infermieri, e di individuare i fattori che influiscono sulla concordanza. Materiali e metodi: Lo studio di tipo descrittivo ha utilizzato un questionario appositamente costruito, da erogare al paziente e all’infermiere che lo assiste, per valutare i problemi che il paziente presentava all’atto del ricovero in un’unità di degenza del Policlinico universitario Campus Bio Medico. Risultati: Lo studio, effettuato su 55 pazienti e 12 infermiere, ha messo in evidenza che il livello di concordanza infermiere/paziente nella percezione dei problemi del paziente è in media del 39,6% ed è maggiore per i problemi della sfera fisica (51%) rispetto a quelli psicologici (37%) e sociali (31%). Discussione: La percezione del paziente dei propri problemi di salute è un fattore fondamentale per la comprensione, la cooperazione e la partecipazione al programma assistenziale, ma anche la valutazione dei problemi del paziente da parte dell’infermiere finisce per influenzare la profondità, l’ampiezza e la qualità della pianificazione assistenziale. I risultati dello studio forniscono interessanti suggerimenti per la pratica clinica, la formazione e la ricerca infermieristica.
